A financing statement (Form UCC1) is filed to perfect a security interest in named collateral and establishes priority in case of debtor default or bankruptcy.

The Addendum (Form UCC1Ad) and the Additional Party forms (Forms UCC1AP) should only be completed and submitted with the Form UCC1 if they are needed to record additional debtors and/or secured parties.

This UCC Financing Statement (Form UCC1) must be reproduced in a format prescribed by the Uniform Commercial Code, including printing on 8 1/2” x 11” white paper. Submittals that do not conform to the specifications will be rejected.

Please send the original document only. Filers automatically will receive an acknowledgment copy of the accepted document and an acknowledgment letter. To obtain a search certificate or copies of the filed statement, enclose an Information Request (Form UCC-11), for each debtor name.
