		Statement of Information Counter - Location Change: Effective May 11, 2009, the Statement of Information counter on the 3rd floor of the Sacramento office has been closed and moved to the 2nd floor, room 255, where it has been combined with the Uniform Commercial Code counter.

		Customer Alert Regarding Misleading Solicitations: Certain private companies have been soliciting business through mass mailings to corporations and limited liability companies with a solicitation similar to a Statement of Information that is required to be filed with the Secretary of State. Please refer to the Customer Alert notice for additional information.

The following forms are provided in PDF file format and can be viewed and printed from your computer using the most current version of the Adobe Reader (available for free from Adobe's Website). Please refer to the instructions included with the form for filing information and due dates.

STATEMENT OF INFORMATION
-	E-file - File the required Statement of Information for most corporations online. Statements that are not filed online can be mailed or delivered in person to the Secretary of State's Sacramento office.
-	Domestic Stock Corporations (Form SI-200 C) Use if filing the first statement or if changes have occurred since the last completed statement was filed.
-	Domestic Stock Corporations (Form SI-200 N/C) Use if no changes have occurred since the last completed statement was filed.
-	Foreign Corporations (Form SI-350) Use if filing the first statement or whether or not changes have occurred since the last completed statement was filed.
-	Domestic Nonprofit Corporations (Form SI-100) Use if filing the first statement or whether or not changes have occurred since the last completed statement was filed.
