The information includes the complete name of the corporation, corporate number, date of incorporation or date of conversion, status, jurisdiction (place of domicile), mailing address, and name and address of the agent for service of process.

Fictitious business names are not filed with the Secretary of State's office. Fictitious Business Name Statements are filed with the county where the principal place of business is located. Certification/Reproduction
