Candidate Intention And Bank Account Statements

All state candidates are required to file the Candidate Intention Statement (Form 501) with the Secretary of State's Political Reform Division before the solicitation or receipt of any campaign contribution (including loans and in-kind contributions). All candidates for an elective city or county office are required to file the Candidate Intention Statement (Form 501) with their respective local filing officer. (Government Code section 85200)

Statement Of Organization (Recipient Committees)

A Statement of Organization (Form 410) must be filed with the Secretary of State's Political Reform Division by all recipient committees that raise $2,000 or more in a calendar year to influence California's state and/or local elections. In the case of a local committee, a copy of the Statement of Organization must be filed with the local filing officer with whom original campaign statements are filed. (Government Code section 84101) An amended Statement of Organization must be filed at the same locations within 10 days of a change in any of the information contained in the most recently filed Statement of Organization. (Government Code section 84103)

Termination Form

A Statement of Organization (Form 410) with the "Termination" box checked must be filed by recipient committees in order to terminate ongoing filing obligations. The original and one copy of the terminating Statement of Organization must be filed with the Secretary of State's Political Reform Division. In the case of a local committee, a copy of the terminating Statement of Organization must be filed with the local filing officer with whom original campaign statements are filed. (2 CCR section 18404)

Campaign Statements

Statewide elected officers; Members of the Legislature; Supreme Court justices, Court of appeal justices, and superior court judges; candidates for those offices and their controlled committees as well as committees formed or existing primarily to support or oppose the offices above shall file a campaign statement by online or electronic means as specified in Section 84605, and shall file the original and one copy of the campaign statement with the Secretary of State.

State candidates committees who do not file online or electronic statements with the Secretary of State and/or have a local committee shall also file a copy of the campaign statement with their county of domicile.

Statewide measures, or the qualification of state ballot measures; and all state general purpose committees including major donor and independent expenditure committees shall file a statement by online or electronic means, as specified in Section 84605, and shall file the original and one copy of the campaign statement with the Secretary of State.

Elected members of the Board of Administration of the Public Employees' Retirement System, elected members of the Teachers' Retirement Board, candidates for these offices, their controlled committees, and committees formed or existing primarily to support or oppose these candidates or elected members shall file the original and one copy with the Secretary of State, and one copy shall be filed at the relevant board's office in Sacramento
