Bank account information reporting by all committees: Effective January 1, 2013, pursuant to AB 481 (Chap. 496, Statutes of 2012), all committees must list the bank account number used by the committee, and the name, phone number and address of the financial institution on the Statement of Organization (Form 410). (This requirement previously applied only to candidate-controlled committees.)

Committees paying for the qualification of a state ballot measure must provide the Attorney General's Title and Summary tracking number for not-yet-qualified measures on their Statement of Organization: Primarily formed ballot measure committees, and general purpose committees spending $100,000 or more, supporting or opposing qualification of a ballot measure are required to identify the measure being supported or opposed and to report those activities. Pursuant to a Fair Political Practices Commission (FPPC) regulation 18410, committees should use the tracking number for measures seeking title and summary, which is issued by the Attorney General's office, to identify the not-yet-qualified measure being supported or opposed on its Statement of Organization (Form 410). (Once the measure has qualified for the ballot, and a number has been assigned to the measure by the Secretary of State, the Statement of Organization is required to be amended to include the assigned ballot measure number.)

Naming the jurisdiction where a committee is active: Pursuant to a Fair Political Practices Commission (FPPC) form change, the Statement of Organization (Form 410) will now include a field to list the "jurisdiction" where the committee is active, which might be a city, county, multi-county or statewide jurisdiction. (Previously, the form specifically asked for the "county" where the committee is active, if it was different from the county where the committee was "domiciled" (headquartered).

Email addresses for committees: Pursuant to a Fair Political Practices Commission (FPPC) regulation 18410, committees should provide an email address on the Statement of Organization (Form 410).
