A National Financing Statement Amendment (Form UCC3) is used to change or add to information that was previously filed on the UCC1 form. This amendment form includes continuations, assignments, terminations, and amendments. Certain combinations of amendments can be made on one amendment form.

This office will only accept the National Financing Statement (UCC1) and the National Financing Statement Amendment (UCC3) and the California revised National filing forms, which black out the social security number boxes. The Addendum (UCC1Ad and UCC3Ad) and the Additional Party forms (UCC1AP and UCC3AP) should only be completed and submitted with the UCC1 or UCC3 if they are needed to record additional debtors and/or secured parties.

This National Financing Statement Amendment (Form UCC3) must be reproduced in a format prescribed by the Uniform Commercial Code, including printing on 8 1/2" x 11" white paper. Submittals that do not conform to the specifications will be rejected.
